Why use a change management checklist? Management of changes to processes or related infrastructure is vital for the best outcome. The objective of the best change management processes is to minimise the number and impact of any related incidents upon related services or processes.
This change management template lists the considerations you need to make in order to create a change management form or process.

Is there a summary of change on the change request form?

What is the reason for change?

What are the benefits to be delivered by change?

What are the consequences if the change is not delivered?

Are there available work around. Have alternative solutions been considered?

What are the business areas, services and people affected by the change?

What systems, infrastructure and interfaces components affected by change?

What are the communications requirements i.e. how will the change be communicated and to whom?

What are the risks incurred via making the change?

What is the back:out strategy for the case of a failed change request implementation?

What is the time schedule and date required for the change request to be implemented?

What is the ballpark estimate for time to evaluate change request submitted and approved?

Is the estimate ballpark for the change request appropriate enough to go ahead with an evaluation?

What type of resources are required to implement the change request (i.e. what kind if people)?

What type of resources are required for the change request (i.e. what type of materials)?

What is the estimated work effort required by resources in hours for the change request?

What is the estimated financial cost of effort required by resources?

What the the estimated costs of other materials?

What is the impact of this change request on other projects?

Have any assumptions been made in formulating the change request and are there any additional notes?

Are there any additional supporting documents supporting the change request?

Are there any additional comments?

What is the date the change request was evaluated?

Was the change request approved or rejected?

What was the approval or rejection reason given for the change request?

What approval restrictions or conditions for the change request?

What was the approval or rejection date for the change request?

Who is the person or body approving or rejecting change request?

Who are the involved change request reviewers?
